# How do I order FVT

## FVT can be ordered through the MyOme Provider Portal

FVT is easy to order through the MyOme Provider Portal:

1. Log in at[https://providers.myome.com/](https://providers.myome.com/)
2. Select “Family Variant Testing”
3. Enter the order (RQ) number of the original tested individual (proband)
4. Complete the test requisition with relevant clinical and family details
